Heart failure service

The heart failure service is designed for those people who have impaired heart function which results in symptoms of breathless and fluid retention.

Patients can be referred to the service by their family doctor or consultant. The service sees patients who have been admitted to hospital and also outpatients.

It involves:

  • A detailed assessment of symptoms and construction of a treatment plan

  • Monitoring of medication effects and adjustment of doses

  • Education

  • Support for the patient and family

The team consists of a senior cardiac clinical nurse specialist and consultant cardiologist who has overall responsibility for the service. Assistance from pharmacists, dieticians and clinical psychologist as indicated.
