Children's home care team at Epsom Hospital

The children's home care team (CHCT) at Epsom Hospital works with children, families, carers, nursery and school staff to enable children to attain and maintain their optimum level of health within their home/school environment.

The CHCT supports and helps parents/carers to care for their sick or injured child at home, reducing trauma caused by hospital admissions and attendances and ensuring as little disruption as possible to the child's/family's routine.

We provide skilled, high quality nursing care, advice and support for families who have children aged 0-16 years with acute or chronic conditions including palliative care, and continuing care for complex needs children/adolescents aged 0-19 years.

The team is comprised of:

  • Children’s community nurses

  • Paediatric diabetes nurse specialist

  • Community sister

  • Nursery nurse

  • Team secretary

We visit children in the following localities in Surrey:

  • Effingham

  • Bookham

  • Fetcham

  • Leatherhead

  • Ashtead

  • Cobham

  • Worcester Park

  • Ewell

  • Stoneleigh

  • Epsom

  • Epsom Downs

  • Dorking

  • Kingswood

  • Woodmansterne

  • Chipstead

  • Banstead

  • Tadworth

  • Walton on the Hill.

Referrals

We accept referrals from GPs, health visitors, school nurses, social workers, paediatric A&E, children's outpatient department, consultant paediatricians, dieticians, therapies and medical teams from other hospitals including RMH/GOSH.

The team requests that written referrals are made so that details can be recorded, although referrals can be received by telephone initially.
