The children's home care nursing team at Queen Mary's Hospital for Children (QMHC) provides homecare to children and young people aged 0-18 years who are registered with a Merton or Sutton GP.
The aim of the children's home care team is to enable early discharge for children from hospital to home by providing nursing care support and advice and to reduce the need for readmissions to hospital.
The homecare team nurses provide home visits and care for clients with a range of conditions i.e. wound dressings, asthma, constipation, eczema, enteral feeding, and home administration of IV drugs - to more severe cases ie children with severe and complex needs, oxygen dependant, children with tracheostomies or ventilated patients.
We also care for children and young people with cancer, palliative and end of life care. The team are involved in providing support and arranging care packages for many of these children This includes health needs assessments, funding applications from the PCT and organisation and training of home carers.
We work closely with families, social care, education, therapists, GP's consultants and voluntary agencies in ensuring all required services and equipment are in place across all aspects of the child's life prior to discharge.

Unfortunately, we do not accept self referrals from patients but would advise them to request a referral from their GP health visitor or school nurse.
